Entry Forms 2008

The entry forms for 2008 are available as a SINGLE 10-page document. This includes the RULES & PAYMENT FORM. Please download this and print as required.

ENTRIES CLOSE ON TUESDAY 29th APRIL - 5pm SHARP

STEP 1
Download and print the forms

Right-click on the link above and choose "Save Target As..." - save the PDF document (called entryforms08.pdf) on to your computer. When the PDF document has downloaded you can print this. You will require Adobe Acrobat Reader to open the document.

STEP 2
Please read the RULES and REGULATIONS before you complete entry form(s).

STEP 3
Complete the entry forms using BLACK CAPITAL LETTERS

STEP 4
Calculate total entry fees that are due:

Individuals: €2.50 per athlete per event (ie 1 athlete doing 3 events => 3 x €2.50 = €7.50).

Relays: €10 per team. Only 1 relay team per age group.

Affiliation fee of €10 per school - this is mandatory for insurance purposes.

STEP 5
Complete the payment form fully. This is where you specify relay teams also.

STEP 6
Enclose payment with completed forms and post to Greg Ashe. Entries will not be accepted by fax, email or phone.
