Entry Forms 2010

ENTRIES CLOSE ON FRIDAY 7th MAY - 3pm SHARP

STEP 1
Download and print the forms (10 pages in total)

Right-click on the link above and choose "Save Target As..." - save the PDF document (called entryforms2010.pdf) on to your computer. When the PDF document has downloaded you can print this. You will require Adobe Acrobat Reader to open the document.

STEP 2
Please read the RULES and REGULATIONS before you complete entry form(s).

STEP 3
Complete the entry forms using BLACK CAPITAL LETTERS

STEP 4
Calculate total entry fees that are due:

Individuals: €2.00 per athlete per event (ie 1 athlete doing 2 events => 2 x €2.00 = €4.00).

Relays: €5 per team. Only 1 relay team per age group.

Affiliation fee - if you have not paid (check list of affiliated schools) then download affiliation form and include with fee with entries.

STEP 5
Complete the paperwork fully and legibly. DO NOT SEND PHOTOCOPIES.

STEP 6
Enclose payment with completed forms and post to Greg Ashe.
Entries will not be accepted by fax, email or phone.
